Some patients with primary hyperparathyroidism developed a peculiar type of bone disease known as osteitis fibrosa, characterised by increased generalised osteoclastic bone resorption, particularly involving the phalanges (causing subperiosteal resorption) (Fig. 18.10) and the skull (causing the appearance known as salt and pepper skull). This presentation is rarely seen nowadays.
